Association of echocardiographic measures of right ventricular diastolic function with functional capacity using the six minute walk test among patients who underwent total Tetralogy of Fallot Correction.

Introduction: Tetralogy of Fallot is the most common cyanotic congenital heart disease and is amenable to surgical repair. Right ventricular function has been shown to be a major determinant of clinical outcome in congenital heart disease patients and RV dysfunction is a significant problem in post operative TOF. TDI allows quantitative assesment of both systolic and diastolic function by measurement of myocardial velocities.
Objective of the study: The study aims to determine the association of right ventricular diastolic function with exercise capacity among pediatric patients who underwent total TOF correction.
Methodology: This cross sectional study done in the Philippine Heart Center from July to December of 2013. Included in the study are all pediatric patients who underwent Total TOF Correction for at least 1 year. All the subjects included were evaluated by medical history, and physical examination during their consult at the OPD. A 6 minute walk test was done to asses their functional capacity and a 2d echocardiography with TDI was also done. Statistical analysis was done using the Independent T-test.
Results: Thirty patients were included in the study. No significant difference was found between those with normal and abnormal 6 minute walk test and diastolic function measurements in Independent T- test. Three patients had E/A reversal in their TDI and also had an abnormal six minute walk test finding. Based on the pearson correlations, a negative linear relationship was found between deceleration time and six minute walk test and a positive linear relationship was found between TDI sep E and 6 minute walk test findings.
Conclusion: Tissue Doppler echocardiography may be used as a non invasive diagnostic tool for post operative patients particularly TOF patients for the early diagnosis of right ventricular dysfunction.
