BACKGROUND: To determine the mortality rates for groups of patients undergoing cardiac valve surgery and to identify risk factors influencing early mortality.
Design: Retrospective Study
METHODS: A total of 150 patients who underwent cardiac valve surgery at the Philippine Heart Center ages 19 years and older who were discharged between January 1, 2003 and June 30, 2004 were classified into 8 major groups and significant risk factors for in-patient mortality were identified. Statistical significance were done using Fisher exact test or Kruskal-Wallis test with level of significance set at p<0.05.
RESULTS: The over all in-hospital mortality rate for patients undergoing cardiac valve surgery was 8.7 percent (13 out of 150 patients). It is notable that mortality rate for multiple valve operation is considerably higher than that of single/isolated valve operation. A pulmonary artery pressure of 60 mmHg and above has a mortality rate of 17.5 percent compared with 3.2 percent for patients with a PASP <60 mmHg. A cardiopulmonary bypass time longer than 3 hours, 2-3 hours, and less than 2 hours has in-hospital mortality rates of 26 percent, 10.7 percent, and 1.4 percent respectively. A cross-clamp time of more than 2 hours has
mortality rate of 16.7 percent, compared with a cross-clamp time of 1-2 hours which is 9.2 percent, and less than 1 hour which is 0. Perioperative renal failure has in-hospital mortality rate of 71.4 percent compared with 5.6 percent early mortality rate of those patients who did not suffer perioperative renal failure.
CONCLUSION: This study identified the risk factors that highly influenced early mortality. A PASP of 60 mmHg and above, longer cardiopulmonary bypass time, long cross-clamp time, and perioperative renal failure were significantly correlated with in-hospital mortality. (Author)
